All new enrollments are by appointment only.
If ordered by the court, it is important to contact us prior to the date noted on the court referral.

If enrolling via a DMV Administrative Per Se, please provide documentation.

To enroll, provide a copy of your Driver’s License, Passport or State ID, and complete the following forms and submit to info@sdsuduip.com (refer to the forms page):
